Initial Nursing Assessment $200
Comprehensive, in-person assessment conducted in your home or assisted living residence. We’ll review your health history, medications, current concerns, daily needs, and personal goals. Following the visit, you’ll receive personalized recommendations for continued services, care coordination, or appropriate follow-up.
Hospital-To-Home Transition Visit $150
Discharge review, medication reconciliation, and recovery assessment.
